2020 № S5 Healthcare information systems and organizational model of care: current situation and opportunities for progress
The paper discusses possible improvement of healthcare through extension of its organizational model and better utilization of medical information systems. A brief review of ‘classical’ healthcare informatization tools is given with their relation to the steps of healthcare workflow that they intend to optimize. All consumers of healthcare are divided in four target audiences based on their needs in care and health preservation. Concepts of ‘health continuum’ and ‘continuum of care’ are discussed, with special focus on the parts of care continuum where informatization is currently lacking. The underlying reasons for such gaps are limitations of the prevailing healthcare organizational model. The current biomedical diagnosis-centric model of care is compared with biopsychosocial person-centric model.
Further progress of healthcare through wider application of biopsychosocial model should expand opportunities for informatization. -

in a medical information system are considered. The problem of the optimal multi-day route for the provision of medical procedures to a patient is solved, taking into account the doctors’ work schedule. Cases of one- and multi-day service are considered. A description of the algorithms is given and their comparison is performed. -

Approaches to ensuring the safety of patient treatment in a medical organization are considered on the example
of surgical treatment. The proposed case is implemented on the basis of the JCI IPSG4 “Ensure Safe Surgery” standards and includes the implementation of preoperative verification and timeout by means of MIS. The interface is considered as a chain of interconnected checklists, which was named “Green wave of patient safety during surgical treatment”. -

2020 № S5 Features of informatization of surgical departments of a medical organization. Planning and management of operational activities in a multidisciplinary hospital using MIS Interin
The article is devoted to the automation of the process of planning and management of operational activities in a
multidisciplinary hospital – the basic principles and features, parameters and factors affecting the formation of the operation plan are formulated, the model of the business process is described and criteria for its effectiveness are given, the methodological foundations for organizing the planning of operational activities are shown – shown the leading role of the relationship between the plan of operations and the plan of hospitalizations, the role of the medical information system in the automation of operational activities is determined.
All identified methods of increasing the efficiency of the operational activity of medical institutions by means of MIS were tested on the example of using MIS Interin PROMIS Alpha at the Federal Research and Clinical Center of the FMBA of Russia. The results that were achieved as a result of the practical application of these methods with the use of MIS in the organization of the daily process of the activities of surgeons in the Federal Research Center of the Federal Medical and Biological Agency of the Federal Medical and Biological Agency of Russia are considered. -
2020 № S5 Patient-centered online platform as a service of digital healthcare ecosystem
Opportunities of healthcare informatization tools are closely related to and limited by requirements of the corresponding
model of care. The paper covers specific features of biopsychosocial patient-centered (BPC) model and their implications for informatization. Some approaches to automation of BPC model are discussed: ecosystem architecture and patient-centered online platform that provides communication infrastructure for consumers and providers of health services. Operation principles, functions and services of the platform are described that should help the platform meet the requirements of BPC model. The proposed online platform can supplement the existing healthcare informatization tools in addressing the challenge of chronic diseases epidemic. -
2020 № 4 Web Application “Congenital Malformations”: Program Effectiveness Evaluation Based on Feedback
Currently, Internet resources are widely used in medical education, becoming one of the key tools of e-learning. We
have developed a web application for congenital malformations and anomalies for medical students as an additional tool for self-learning. The web application contains two components: multimedia descriptions of congenital malformations, including images, animations, videos and interactive graphical tests; and the knowledge control module. It is important to evaluate effectiveness of web application and to improve the quality of the resources. We sought to evaluate the effectiveness of a web application by analyzing user knowledge dynamics and use the information to improve content. The anonymous testing involved 260 users – doctors, medical students and teachers. Using the longitudinal method of the study, we analyzed the dynamics of the group-averaged rate of correct responses after repeated testing attempts. The results showed differences in the initial level of knowledge of users, and the results of medical students’ tests in dynamics were improved more than the results of doctors.
The initial value of the percentage of correct responses to the control questions and the growth dynamics of this indicator after repeated attempts are important indicators for assessing the usefulness of a particular question. For developers, this information, based on objective indicators, has proved useful for improving the educational resource. -
2020 № 3 The role of health information technology in promoting patient safety
The paper reviews current scientific evidence on the impact of different health information technologies (HIT) on patient
safety, as well as the potential negative consequences of HIT introduction, including systems of computerized physician order entry(CPOE), clinical decision support (CDSS), medication administration technologies, telemedicine and telemonitioring, electronic incident reporting and electronic medical records (EMR). It was concluded that the most convincing evidence of effectiveness inimproving patient safety, reducing the risk of medical errors and healthcare-related adverse events, have CDSS and technologies that incorporate decision support elements: CPOE, EMR and telemonitoring. -
2020 № 2 Evaluation of innovations for healthcare
Actuality : evaluation system for healthsaving technologies innovation level development is defined by need of innovative healthcare development strategy implementation and insufficient definition of innovative healthcare technologies criteria. Implementation of new healthcare technologies into systemic healthsaving requires managerial decision making support for innovations integration.
Goal: healthsaving technologies innovation level multicriteria evaluation scientific approval and development.
Materials and methods: program includes systemic and content analysis, expert evaluation, analytical and statistical methods. Working with Public Health experts focus group three steps algorithm was used. Excel based data processing was performed with indicative indicators and indicative space calculation.
Results: healthsaving technologies innovative level multicriteria analysis method was designed. Method novelty consists of first time introduced for healthcare innovative level criteria, expert card and evaluation algorithm. Healthsaving technologies final evaluation indicative indicators and space were calculated as a base for managerial decision choice support for their integration.
Conclusion presented evaluation scale allows medical technologies ranging according to needs of solutions for certain tasks. -

security, and reliability of the collection and storage of information. Since 2017, in the Federal State Autonomous Institution
Scientific Research Center of Neurosurgery named after ac. N. N. Burdenko of the Ministry of Health of Russia (NMRC of Neurosurgery) the clinical data management process was modified and a special management information system was introduced
into the routine research practice.
This article opens a series of articles on clinical research management at the NMRC of Neurosurgery.

Prolonged monitoring of the patients health is the important issue in the treatment of patients after allogeneic hematopoietic
stem cell transplantation (allo-HSCT) and is extremely important for the transplant team. The basis of this awareness is the
aggregation, systematization and analysis of data about what happens to the patient outside the hospital, namely those parameters
that are clinically important for physicians. The basis of the future system of patients’ monitoring is a self-monitoring portal for
patients after allo-HSCT. These arrangements will reduce the time of patients’ survey on the following examination and potentially
lead to an increase in overall survival due to timely hospitalization, which in turn will increase the effectiveness of medical care. -
2019 № 3 The use of a clinical information system for patients with circulatory system diseases selection to supply them a high-tech medical care

Abstract. The manuscript presents results of the clinical information system (CIS) application for ensuring the selection of patients with circulatory system diseases for receiving them of high-tech medical care.
The using of CIS is based on the level of morbidity, hospitalization, mortality of the population analysis at Moscow region. CIS provide specialists the information about comparative analysis of population health indicators to determine the needs of municipal
citizens in specific types of high-tech medical care in relevant medical organizations.
Also CIS can be used for monitoring the selection of patients for high-tech medical care, assessing the quality and effectiveness of this type of medical care in various medical organizations. Measures in public health conducted at Moscow region led to decreasing the death rate of patients with circulatory system diseases (CSD). This rate has decreased from 676.2 per 100 thousand population in 2015, to 475.2 per 100 thousand in 2018.
We should pay attention that such data demonstrates the lower mortality from CSD than the average in the Russian Federation and the Central Federal District in recent years. -

Formal models of these rules are described. A comparative analysis of the risk classes of SaMD is done, examples of which are described in the documents IMDRF, defined by these rules.

2018 № 3 Automation of the medical supply system. Database of medicinal means of a multi-disciplinary medical organization
The modern period of development of public health services differs by wide introduction of information technologies in administrative and clinical practice. The implementation of activities in the era of rapid digitalization, the high intensity of work in conditions of multi-channel financing, increasing competition and the difficult situation with tariff policy within the framework of territorial programs of state guarantees, accompanied by accelerating the pace of changes in the regulation of economic processes, raise new, higher requirements for the level of management systems of medical organizations and their respective automation.
The results of the analysis of the key supporting processes of medical organizations, such as drug supply, reflect the general dissatisfaction with the current and outdated drug delivery management system, its low ability to respond promptly to external and internal challenges. The professional community clearly needs to find and develop innovative approaches to improving the drug supply system of a medical organization using automated control systems.
Within the framework of this article practical approaches to the automated management of information flows related to the movement of medicines in the process of resource provision of a multi-profile medical organization are described on the example of the federal state budgetary institution “National Pirogov Medical Surgical Center”of the Ministry of Healthcare of the Russian Federation (hereinafter Pirogov Center). -
2018 № 2 Prospects for the further development of the medical statistics service through the transition to management based on data
In the article the problems of the system of collection of state statistical reporting existing in Russia and the consequences to which they result are analyzed. A review of literature and publications in the media and the blogosphere is provided, which reveal the existing shortcomings of medical statistics.
The gradual development of health management based on data with the refusal to use statistical reports is proposed. The key idea is a gradual refusal to apply the existing forms of state statistical reporting. Instead, it is necessary to create and consistently develop a system of support for management decisionmaking in the health sector, presented as one of the federal components of the EGIS. Ultimately, such a system should completely replace state statistical reporting approved by the orders of Rosstat, Ministry of Health, FFOMS and other executive authorities, including regional government bodies, as well as numerous disparate “monitoring”, “registers” and other federal management systems available in present time. The established single federal service EGISP should accumulate in itself all the formalized primary data, primarily depersonalized, from other components of the EGISP, such as medical information systems of medical organizations (MIS MO), regional services and EHISM systems, information systems of the TFOMS and etc. -

2016 № 6 A Project for the Creation of a Management Information System for a Large Multidisciplinary Healthcare Organization. The Results.
The paper presents the experience of building the Information Management System of 9 Medical and diagnostic center of the Russian ministry of defense. Building integrated solutions for medical institutions included in a medical organization is not a trivial task; the build process takes a long time, technologically and knowledge-intensive. The generalization of this experience, as well as a description of some key decisions are the subject of this article. The paper concludes that it is possible to use the created system configuration as a model for military health care facilities.
