  • 2023 № 1 Planning medical care for patients with cancer in the constituent entities of the Russian Federation.

    National project “Fight against oncological diseases 2019 - 2024” implemented for the fourth year. During this time, experience has been gained in planning the volume of medical care, including diagnostic tests, and financial support in the system of compulsory medical insurance, but there is no systematic analysis of planning problems. The purpose of this study was to study the planning by the subjects of the volume of medical care and their implementation of the target indicators of the territorial programs of state guarantees of free medical care (TPGG) and to identify planning problems. We analyzed the territorial programs of state guarantees of some subjects of the Russian Federation. Also, from the territorial CHI Funds (TFOMS) we received information on the performance of TPSG indicators: the
    volume of care in round-the-clock and day hospitals, planned and actually performed molecular genetic studies and cases of positron emission tomography combined with computed tomography in 2021 and the first half of 2022 of the year. We found significant problems, some of which have causes at the federal level, others are purely regional in nature, which lead to significant differences in planning the volume of medical care between the subjects of the Russian Federation. Conclusion. Planning the volume of medical care in the field of “Oncology” varies significantly between the subjects of the Russian Federation and has systemic difficulties, control over the effective use of funds is insufficient. We consider it necessary when planning medical care for the oncology profile in the Program of State Guarantees of
    Free Medical Care to take into account the standardized incidence rate per 100,000 population, and not the number of insured persons in the subject.

  • 2023 № 2 Analysis of drug supply for patients with malignant neoplasms at the inpatient stage of treatment.

    Drug provision of patients with malignant neoplasms in a hospital and day hospital in the constituent entities and Federal districts of the Russian Federation has not been studied enough. Meanwhile, hundreds of billions of rubles are annually spent on the purchase of drugs for these patients. The aim of the study is to analyze the drug supply of patients with malignant neoplasms at the inpatient stage of treatment. We analyzed the purchase of medicines at the expense of compulsory medical insurance in the subjects and federal districts of the Russian Federation according to the website zakupki.gov.ru. To compare federal districts and constituent entities with each other, we compared the amounts spent on the purchase of medicines at the expense of compulsory medical insurance, in terms of 1 patient with a newly diagnosed cancer in each federal district and constituent entity, respectively. We found significant differences
    in the drug treatment of patients with MN not only between the Federal Districts, but also within the Federal District. Moreover, the difference in drug provision of patients with specific drugs in the subjects reaches multiple values. Oral drugs are also used differently, with a mixed trend in the Federal Districts of their use over the past two years. Drug provision of patients with malignant neoplasms in the constituent entities of the Russian Federation at the inpatient stage of treatment varies significantly, patients have different access to drugs with oral administration.

    The article presents morbidity and mortality in the Moscow (MO) from cancer for improvement organization of specialized medical care with oncology pathology. An analysis is made of the reasons for the excess of the specialized Medical Assistance (MSP) (including high-tech medical care - ВМП) for residents of the Ministry of Defense for malignant neoplasms more than 2 times mean values for the Russian Federation, in a 24-hour hospital due to a larger residents of the Moscow region of the NSR and the VMP at the expense of federal medical centers in Moscow.
