A first approximation of such a solution is proposed – the creation of a list of diagnoses; the limitations of this approach are described.   • 2019 № 1 Maximus Intellectual Information and Analytical System for Medicine and Healthcare

    This article discusses the relevance, purpose, functions and capabilities of intelligent information and analytical modules of diagnostic and treatment processes management systems, on the base of the use of Maximus system in the regional network of clinics of Nefrosovet group, as well as in relation to medicine and healthcare in Russia as a whole. It justifies the importance of automating the data collection, starting from the diagnosis and treatment process to the middle and upper levels of analysis and reporting. Article presents a line of business intelligence software services and decision support systems – process control center, visual analyzer of DTP parameters and reporting system, analytical patient registry, diagnostic machine, causal patient model, diagnostic and treatment programs, etc., using the nephrology service with hemodialysis, treatment of complications and associated diseases as an example.
    The architecture of presented system is service oriented and multiplatform. Article also describes the structures of Maximus BI and DSS subsystems, as well as the architecture of one of the main services – diagnostic and treatment programs. Article shows modern information technologies for managing business processes used in the system, as well as selected user interfaces and solutions.

  • 2019 № 1 Benchmarking for assessing the quality of digitization of radiology departments: methodology development

    Basing on the “Digital Imaging Adoption Model (DIAM)”, systematized international and own practical experience, there
    was developed a tool for benchmarking the level of digitization of radiology departments. The tool includes: 1) a reference “Medviz” maturity model of medical imaging digitization; 2) a basic methodological document (unified five-level digitalization strategy); 3) a digitization degree assessment tool; 4) a structured set of applications. In process of benchmarking, radiology departments are classified in accordance with the author’s Medviz model. Following the established level, a special set of recommendations is formed as basis for further managerial decisions. The methodology was successfully tested by us in five medical centers of the Department of Healthcare of Moscow. The development is applicable for complex studies of IT support of medical centers.

    The article presents the main approaches to the application of a software package for assessing the effectiveness of a new work management of outpatient hospitals by identifying the reasons of patients’ dissatisfaction with primary health care following citizens’ appeals and independent assessment of the quality of service delivery of medical organizations at various levels. The results of the Internet websites evaluation sometimes contain the good results of work of outpatient hospitals, but the number of justified complaints about the dissatisfaction with health care in specific medical organizations remains high. The PC allows to analyze the main causes of patients’ dissatisfaction with medical aid and substantiate specific recommendations on the formation of a new model of the work of medical organizations with the aim of significantly increased population’s satisfaction with the quality of medical care provided in outpatient settings.

  • 2018 № 4 Radiology information system of the Krasnodar region: development of the reference clinical and diagnostic center for breast pathology

    In order to increase the availability and quality of specialized oncological care for the population of the Krasnodar Territory, the clinical and diagnostic Center of breast pathology was created. Teleradiology is one of the most efficient way for remote consultations. The article contains a description of the methodology for organizing the work of the Reference Center using telemedicine technologies via Radiology in-formation system of the Krasnodar Territory (RIS). DICOM modalities in diagnostic departments, local PACS in medical facilities are the part of regional RIS and also include 2 mammography units located in interterritorial oncological dispensaries. Every month new mammography exams are delivered to local PACS, which determines the relevance of their remote consultation. To fully operate the Reference Center as key participant in the system of teleradiology consultations, it is necessary to create a complex diagnostic information system in the regional clinical oncology dispensary. This is one of the results of the paper. The use of a unified system increases the efficiency of the information technologies in a remote mammographic screening and also allows standardizing and formalizing activity of a radiologist according to recommendation such as BIRADS, facilitates and speeds up the routine work of the radiologist, improves the quality of radiology interpretation, provides to radiologist effective help, using computeraided diagnostic

    Kemerovo Region limb amputations register formation aim is to improve life expectancy and quality of life among patients (disabled persons) with limb loss on the ground of advancements in rehabilitation and healthcare delivery for this population.
    This paper determines the primary organisational and technical challenges that should be met when formatting the limb amputations register as well as the solutions to address them. It establishes the list of system credentials for limb amputations database creation; determines the sources of primary data; describes the building of the organisational system of primary data gathering and updating. Designed amputations register formation technology is considered as a basis for implementation of the registration of patients with socially significant pathology in the region.

    In article the concept of the information and analytical system developed on the basis of a neurology unit of the Central regional hospital of the Fish and Suburban district of the Republic of Tatarstan and intended for ensuring activity of School of a Stroke is developed.   • 2017 № 3 Online method medical-economic control of prescriptions for preferential drug provision

    There is a high probability of making mistakes (intentional and unintentional) while filling the electronic form of the recipe for preferential drug provision, that may lead to violations in the prescription of drugs. Expert detection of prescription form filling errors after the drugs had been released to the patient in pharmacy can’t prevent the negative impact on patient’s health. The purpose of the study is to improve the effectiveness of organizing and performing the medical control of drugs prescribing, to simplify procedures for providing citizens with preferential medicines, applying the developed methods and algorithms for real-time verification of the selected recipes. The paper represents the method and the development of decision making support algorithm to be used while performing the medical expertise, also called medical -economic control. The opportunities of unified information system for prescribing and releasing of preferential recipes in the Krasnodar Region are observed in the paper in the frame of implementation of the “electronic prescription” technology and medical expertise of preferential drugs prescribing in real time. The prospects of medical expertise performing for detecting cases of polypharmacy in advance are established in cases of multiple drugs prescription (more than five). The importance of performing medical expertise was defined for all kinds of prescription, not only for preferential drugs, consequently the suggested method and developed algorithm should be applied to medical expertise of all kinds of electronic prescription in real-time

    Annotation. This paper proposes a way to automate the monitoring of pregnant Tula region taking into account the peculiarities of the organization of obstetric care in the region. Designed for automated functional tasks pregnant monitoring system allows us to implement a three-tier curation pregnant women taking into account the performance of the existing legal and regulatory aspects of the provision of obstetric care. A model of the structure of the monitoring system of obstetrics, automates the process of supervision of pregnant women at all levels of monitoring, automated processes for evaluating and monitoring perinatal risks.
